I hereby agree that I am voluntarily participating in a glass art workshop which has been planned and provided by Kate Civiero/Infinite Glassworks.

I am aware that this workshop may include the use of potentially dangerous materials, tools and equipment at varyingly high levels of heat. I recognize and acknowledge that glass art may be considered hazardous and that voluntarily participating involves inherent risk and danger. I fully understand and will apply the oral and written safety rules and precautions necessary for the requirement of my participation in said workshop.

I recognize and EXPRESSLY ASSUME the potential risks associated with a workshop in glass and I hereby release and discharge Kate Civiero, Infinite Glassworks, and/or any of its employees, from any and all claims, liabilities, obligations, damages, losses and medical claims which may arise out of my participation in the above mentioned activities, including but not limited to claims for delays, cancellations, or injuries.

I am in good health and have no physical limitations which would impair my safety or the safety of others at this facility. At any time, if I am uncomfortable with my knowledge or experience level, I will request personal supervision or assistance.

I further acknowledge by submitting this form that I have read this waiver carefully and understand its terms.
